On Saturday, December 17, 2011, a surface mechanic was seriously injured while setting a transmission into a CAT 777F rock truck using a truck-mounted hydraulic crane. The crane suddenly failed, allowing the boom to fall, striking him in the head and knocking him down between the frame rails of the truck. The victim was standing on the frame rail of the rock truck at the time of the accident.

One of the most common questions I used to get on emergency response scenes is "how do you determine when we can stop wearing SCBAs and begin wearing Air Purifying Respirators".  The decision is often a heated debate, due to the users lack of knowledge.  The scary part, these are trained HAZMAT Technicians who should know better.  But a quick survey of some clients and SAFTENG members I found there are a lot of folks out there who may not fully understand the decision logic behind the type of respiratory protection is needed.  So here we go... Another how NOT TO RESPOND video (Ammonia Tanker)

A NH3 Tanker Truck rolled over and lost it's load.  A GREAT video to see the behavior of a Liquid Spill of NH3.  We can see the liquid pouring our of the tanker and immediately flashing off to a gas.  Auto-refrigeration is observed as the gas hangs low over the pooling liquid.  AMAZING FOOTAGE at 1:53 into the video and another CRITICAL mistake at 2:06 into the video.

I am not positive that this is a legit list, as there is NOTHING new on this org's website since 2009!  But all the incidents are legit as I covered them in the Incident Alerts.  Not sure who or what org put this together, but it it relevant info for safety discussions.  From agriculture to mining to health care, the occupational fatalities that made the 2011 top ten list compiled by the National Council for Occupational Safety and Health's (COSH) spanned many industries and highlighted notable gaps in regulatory attempts to address workplace hazards.

A new fuel tanker arrives on location somewhere in the Middle East.  The American HSE manager tells the fleet supervisor (Middle Eastern with broken english) to ensure that the tanker is clearly labeled "Diesel Fuel" and "No Smoking" in Arabic.
