2016 Video of the Week #41 (LOTO Fatality - Hydraulic Press)
WARNING! Viewer Discretion is STRONGLY ADVISED. This video is a CCTV clip showing a worker dying in a hydraulic press, no blood or guts; but you see a man die before your eyes. He died from compression asphyxiation. We can see the press has serious safety issues (no two-hand controls or light curtain). I wonder how many times this worker has run this press? He looked awfully smooth right up to the point where he failed to lock it out to SET IT UP for the next run. |
