25 years old.  Less than a week on the job!  She was CLEANING a machine which grabbed her ponytail and pulled her in.  She could not reach the E-Stop!  OSHA is investigating.

The first thing that pops into my mind regarding something like this is a lack of training. On the surface, the fact that she'd only been there a week and didn't have her hair tied up and secured seems telling.
