Tip:  This is a general industry facility.  The workers are contractors.  The vessel is part of a PSM/RMP covered process.  The vessel manway is only 16" in diameter.

Safety shower not readily accessible.
What is in the blue drums inside of the containment?
if sand blasting - Hot work on PSM area?
Housekeeping - random aerosol can and bottle of Perrier ;-)

Sorry for the confusion... to clarify the situation:
1) the 16" manway is on the other side of the vessel out of sight. It was too small to use for the work scope so the bigger hole shown in the photo was made.
2) part of the problem with the hole that was cut - it removed the NFPA 704 label on the tank.

i think the hole is bigger than 16"?? I agree with Tim also if we knew what type of chemical it would probably have more regulatory requirements.

Being in Australia and not being familiar with Osha, but having an understanding of what these guys are doing;
1. No Confined space entry sign visible on vessel,
2. No 'Danger - Hydroblasting' sign visible.
3. person has broken the plane of the Confined space without a permit being displayed at the entry, 4. Safety Standby person not monitoring.
