What is in the blue drums inside of the containment?
if sand blasting - Hot work on PSM area?
Housekeeping - random aerosol can and bottle of Perrier
1) the 16" manway is on the other side of the vessel out of sight. It was too small to use for the work scope so the bigger hole shown in the photo was made.
2) part of the problem with the hole that was cut - it removed the NFPA 704 label on the tank.
1. No Confined space entry sign visible on vessel,
2. No 'Danger - Hydroblasting' sign visible.
3. person has broken the plane of the Confined space without a permit being displayed at the entry, 4. Safety Standby person not monitoring.
